Role Description
The Patient Care Coordinator (Account Manager) is the liaison between the senior communities, patients, and the practitioners. The Account Manager will be responsible for:
• Creating/finalizing practitioner schedules.
• Managing relationships with key decision makers within the community or facility.
• Ensuring the successful implementation and utilization of our services.
• Maximizing the provider’s visit at the facility while ensuring accurate patient information is documented.
• Understanding insurance eligibility and requirements for referrals or pre-authorizations.
Qualifications
• High School Diploma (or equivalent).
• Prior Healthcare Experience: Background in patient care coordination, patient scheduling.
• Strong understanding of insurance eligibility using Availity and Payor specific platforms.
• Outstanding Communication: Friendly, clear, and professional phone manners.
• Organizational Proficiency: Able to handle multiple tasks seamlessly and accurately in a fast-paced environment.
• Tech Savvy: Comfortable navigating medical software and EHR systems.
• Compassionate Mindset: A genuine desire to help others and prioritize patient care.
• High-speed internet access.
Requirements
• Notify communities/facilities and providers of scheduled visit dates, request consultations, and provide visit schedules.
• Organize and maintain monthly provider calendars.
• Create visit templates and reports with scheduling software for monthly flyer distribution to accounts.
• Develop and maintain strong relationships with key decision makers within the community/facility to facilitate growth and address any concerns.
• Identify areas of concern and opportunities for growth.
• Maintain updated account information in CRM.
• Work closely with practitioners to ensure seamless patient care.
• Collaborate with internal teams to ensure customer satisfaction and address any issues that arise.
• Complete tasks assigned in Planner.
• Schedule and confirm patient appointments, follow-ups, and referrals.
• Create and upload schedules for practitioners and facilities.
• Coordinate with patients, Power of Attorneys, and facilities to schedule appointments or address concerns as needed.
• Using the CRM, notify the facility of upcoming visits and request patient sign-up list and any missing documentation.
• Update and add contacts as needed.
• Report the number of patients seen at each facility.
• Respond to voicemails in a timely manner.
• Encourage the building of relationships to promote growth within the facility by phone and email.
• Monitor insurance logs for new insurance approvals.
• Monitor outstanding referrals and authorization requests.
• Monitor Reschedule queue in eDerm and reschedule patients as needed.
• Monitor fax folder to confirm all items were addressed.
• Organize practitioner’s calendar in Outlook with assigned facilities and times.
• Template facilities in software and run reports to ensure accuracy for patient scheduling.
• Format reports to create monthly flyers for distribution.
• Collaborate with internal teams; marketing, management, and patient care coordinators to review areas of concern and opportunity for growth.
